Linn Health & Rehabilitation Center Linn Health & Rehabilitation Center East Providence, RI East Providence, RI Part-time Part-time $39 - $42 an hour $39 - $42 an hour Job Description MDS Coordinator Primary Responsibilities Ensure staff implementation of resident plans of care and evaluation of such plans of care Monitor resident medical records to ensure that they are kept in proper order Monitor use of psychotropic drug use as per pharmacy recommendations, and ensure timely reductions and AIMS testing every quarter Assessment, development, and informing of both units of all aspects of resident care Formal notification of all parties of scheduled patient care conferences Medical record audit with 48 hours of admission and discharge Responsible for initiation and completion of MDS RAP summaries and RAP guidelines for all residents and final submission of MDS to state as regulations require Requirements and work
Experience:
Must be a registered Nurse with a current Rhode Island licensure.
Must have 3-5 years of recent long-term care experience Must have working knowledge of Medicare/Medicaid and PPS forms of reimbursement along with private payment and other insurance Must be familiar with sub-acute care, skilled nursing care, and non-skilled care Must be familiar with state and federal guidelines and regulations for long term care Must be familiar with and able to implement resident assessments, RAP's, and care plans We offer a competitive salary, a comprehensive and generous benefits package, which includes paid time off, company paid Blue Cross health plan to those eligible, dental, and 403-B retirement plan.
